ABSTRACT

@#A cemental tear is defined as an incomplete or complete detachment of the cementum along the dentino-cemental junction (CDJ) or the incremental line within the body of the cementum, which can also involve part of the root dentine adjacent to the cementum. The pathogenesis of cemental tears is not fully elucidated. From the literature review, possible predisposing factors were identified, including tooth type, sex, age, periodontitis, previous periodontal treatment or root canal treatment, history of dental trauma, and occlusal trauma or excessive occlusal force. The morphology of cemental tears can be either piece-shaped or U-shaped, which usually contributes to periodontal and periapical breakdown. Clinically, cemental tears have a unitary periodontal pocket and present with symptoms mimicking localized periodontitis, apical periodontitis, and vertical root fractures. Imaging examination is of great significance for the clinical diagnosis of cemental tears, which often manifest as thin ‘prickle-like’ radiopaque masses located longitudinally adjacent to the affected root surface. Exploratory surgery is needed in some cases. Although intraoperative cemental fragments and cemental lines on the root surface can assist in the diagnostic process, histopathology examination is the gold standard for the diagnosis of cemental tears. The treatment methods vary depending on the timing of the correct diagnosis and the clinical or radiological manifestations. With the development of regenerative biomaterials and the development of intentional replantation, an increasing number of affected teeth can survive for a long time. The aim of this review is to systematically describe the biological basis and predisposing factors, clinical features, radiographic and histological characteristics, diagnosis and clinical management of cemental tears, and treatment outcomes to help make a clear diagnosis and develop a personalized treatment plan.

ABSTRACT

@#Abstract: Objective To observe the etiological distribution, basic information, clinical characteristics, imaging and pathological features, treatment regimens, and prognosis of pathologically confirmed cases of pulmonary mycosis, aiming to improve the diagnosis and treatment level of pulmonary mycosis. Methods The clinical, imaging and pathological data of patients with pulmonary mycosis diagnosed by pathological biopsy in the Affiliated Hospital of Southwest Medical University from January 2014 to December 2021 were retrospectively analyzed. Results There were 77 cases of pulmonary mycosis who were diagnosed by pathology, and of these patients, 42 cases (54.54%) suffered from pulmonary aspergillosis, 34 cases (44.16%) suffered from pulmonary cryptococcosis, and 1 case (1.30%) suffered from pulmonary mucormycosis. Among the 77 patients, there were 38 male and 39 female patients, with an age range of 25 to 68 years old (mean age 51.13±10.32 years old). The common respiratory symptoms on admission included cough (33 cases, 42.86%), hemoptysis (24 cases, 31.17%), expectoration (22 cases, 28.57%) and chest pain (13 cases, 16.88%). Chest imaging features mainly included pulmonary nodules (37 cases, 48.05%), cavity (14 cases, 18.18%) and air crescent sign (10 cases, 12.99%). In this study, the main treatment measures for pulmonary mycosis were surgical resection (47 cases, 61.04%) and antifungal therapy combined with surgical resection (19 cases, 24.68%). After active treatments, most of these patients (72/77, 93.51%) discharged with better condition. Conclusions Pulmonary aspergillosis and pulmonary cryptococcosis are common pulmonary mycosis diagnosed by pathology. The main respiratory symptoms on admission are cough, expectoration and hemoptysis. Pulmonary nodules are the most common imaging features, and "air crescent sign" can be seen in some patients with pulmonary aspergillosis. Most pulmonary mycosis can have good treatment outcomes. Combining fungal histopathological characteristics and fungal special staining such as Periodic Acid-Schiff (PAS) staining and Gomori methenamine silver (GMS) staining can identify most pathogenic fungi into genera, which has important clinical significance for the timely diagnosis and treatment of pulmonary mycosis

ABSTRACT

ObjectiveTo observe the repair effect of Dahuanglingxian prescription (DHLX) on bile duct epithelial cells of rats. To explore whether its mechanism of action is to adjust the mutual binding of transforming growth factor -β (TGF-β) activated kinase 1(TAK1) and tumor necrosis factor receptor-associated factor 6 (TRAF6), and regulate the activation of the nuclear transcription factor -κB (NF-κB)/mitogen-activated protein kinase (MAPK) signaling pathway. MethodThe 20 SD rats were randomly divided into normal group and DHLX group, 10 rats in each group, were given saline and DHLX (320 mg·kg-1·d-1) for 8 days, to prepare normal serum and DHLX serum. Biliary epithelial cells were extracted from normal SD rats and divided into 9 groups: Normal group, model group (20 mg·L-1), LPS+DHLX group (20 mg·L-1+10% DHLX), LPS+PDTC group (20 mg·L-1+200 μmol·L-1), LPS+SB203580 group (20 mg·L-1+0.5 μmol·L-1), LPS+PDTC+SB203580 group (20 mg·L-1+200 μmol·L-1+0.5 μmol·L-1), LPS+PDTC+DHLX group (20 mg·L-1+200 μmol·L-1+10% DHLX serum), LPS+SB203580+DHLX group (20 mg·L-1+0.5 μmol·L-1+10% DHLX serum), LPS+PDTC+SB203580 +DHLX group (20 mg·L-1+200 μmol·L-1+0.5 μmol·L-1+10% DHLX serum). The microscopic observation of morphological changes in each group of cells after drug intervention. Enzyme-linked immunosorbent assay(ELISA) was used to detect the expression of (IL)-1β and IL-6 in each group of cells. Western blot detected the expression levels of TAK1 and TRAF6 proteins in each group of cells, Co-IP detected the interaction between TAK1 and TRAF6, and further observed the distribution and co-localization of TAK1 and TRAF6 using Laser confocal microscope. ResultAfter the action of LPS, the cell synapses are reduced, the cell body becomes significantly rounded and smaller, but the cell morphology of each group tends to be normal after medication. Compared with normal group, the expression levels of IL-1β and IL-6 in model group were significantly increased (P<0.05), while the expression level of TAK1 was decreased while the expression level of TRAF6 was increased (P<0.05). The content of TAK1-TRAF6 protein complex showed a decreasing trend, and the two proteins co-located in the cytoplasm. Compared with model group, the expression levels of IL-1β and IL-6 in LPS+DHLX group were significantly decreased (P<0.05), the expression level of TAK1 was increased and the expression level of TRAF6 was decreased (P<0.05), the content of TAK1-TRAF6 protein complex was significantly increased (P<0.01), and the two proteins were significantly co-located in cytoplasm. Compared with LPS+DHLX group, the expression levels of IL-1β and IL-6 in other groups were significantly decreased (P<0.05,P<0.01). TAK1-TRAF6 protein complex content in each group was significantly decreased after pathway blocker intervention (P<0.05), while TAK1-TRAF6 protein complex content in each group was significantly increased after pathway blocker combined with DHLX intervention (P<0.05). Co-localization of the TAK1-TRAF6 in cytoplasm was not obvious. ConclusionIn the LPS-induced inflammatory response of bile duct cells, the binding of TAK1 and TRAF6 showed a weakening trend, but DHLX could reverse the phenomenon, we think the mechanism of action may be related to promoting the mutual binding of TAK1 and TARF6 to inhibit the activation of the NF-κB/MAPK signaling pathway.

ABSTRACT

OBJECTIVE: To explore the protective effect of tea polyphenols and its mechanism in potassium dichromate(PD)-induced acute renal injury in mice. METHODS: The specific pathogen free weaned Kunming mice were divided into control group, model group and low-, middle-and high-dose tea polyphenols groups, with 12 mice in each group. Mice in the control group were given 0.9% sodium chloride solution, and mice in other four groups were given PD solution with 4.275 mg/kg body weight every morning by intragastric administration. Then, mice in the control group and model group were given 0.9% sodium chloride solution in the afternoon, while mice in the low-, middle-and high-dose tea polyphenols groups were given 0.3 mL tea polyphenols solution with a dose of 200, 400 or 600 mg/kg body weight, respectively by gavage, once a day for two consecutive weeks. The body mass of mice was weighed during the experiment. At the end of the experiment, the mice were sacrificed. The kidneys were removed and weighed. The kidney organ coefficients were calculated. The levels of urea nitrogen and creatinine in serum were determined by two-point method, the activities of catalase(CAT) and glutathione peroxidase(GSH-Px) in serum of mice were detected by colorimetry. The pathological change of kidney in mice was observed. RESULTS: The body weight of mice in the model group decreased(P<0.05), while the kidney mass, renal organ coefficient, serum levels of urea nitrogen and creatinine increased(all P<0.05), and the serum activities of CAT and GSH-Px decreased(all P<0.05) compared with the control group. The body weight of mice in the three tea polyphenols groups increased(all P<0.05), while the kidney mass, renal organ coefficient, urea nitrogen and creatinine levels in serum decreased(all P<0.05), and the activities of CAT and GSH-Px in serum increased with the increasing intervention dose of tea polyphenols(all P<0.05) compared with the model group. The change of acute renal injury was mainly caused by renal tubular injury in the model group. The pathological changes of renal tissue in the three tea polyphenols intervention groups were improved compared to that in the model group, and the improvement showed a dose-effect relationship with the intervention of tea polyphenols. CONCLUSION: Tea polyphenols have a protective effect on PD-induced acute renal injury with a dose-effect relationship. Its mechanism of action is related to the fact that tea polyphenols can reduce or reverse oxidative stress and inflammation in the kidney.

ABSTRACT

Objective: This study aimed to classify hepatocellular carcinomas (HCCs) according to their diameter using statistic technology and evaluate the prognosis of the classified groups after the combined use of transarterial chemoembolization (TACE) and radiofrequency ablation (RFA). Materials and Methods: Electronic medical records of 128 consecutive patients who underwent TACE-RFA as the initial treatment for HCC from January 2010 to April 2018 were retrospectively analyzed. TACE was initially performed with subsequent RFA performed after 3–7 days. The decision tree model was used to classify overall survival (OS), progression-free survival (PFS), local recurrence rate (LRR), and treatment complications in HCC. Results: The tumors were divided into three groups of sizes ≤2.9 cm, 2.9–4.8 cm, and >4.8 cm. The group of tumors >4.8 cm showed inferior OS, PFS, and LRR than the other two groups (P < 0.05) on long-term follow-up but not in thefirst 6 months (P > 0.05). The groups of tumors ≤2.9 cm and 2.9–4.8 cm showed no statistically significant difference in OS, PFS, and LRR (P > 0.05). Conclusions: The cutoff points of 2.9 and 4.8 cm were achieved using the objective decision tree model rather than the artificial division of 3 and 5 cm. The prognosis was not significantly different between the groups of tumors ≤2.9 cm and 2.9–4.8 cm, and the prognosis of the two groups was better than the group of tumors >4.8 cm in the long-term follow-up but not in thefirst 6 months

ABSTRACT

PUE@PEG-PLGA micelles has excellent characteristics such as small particle size, high drug loading and slow drug release. The results of TEM electron microscopy showed that PUE@PEG-PLGA micelles had obvious core-shell structure. The critical micelle concentration(CMC) of PEG-PLGA micelles determined by pyrene assay was about 4.8 mg·L~(-1). Laser confocal experiments showed that PEG-PLGA micelles can enhance the cellular uptake of coumarin-6 and aggregate around the mitochondria; quantitative results of extracellular drug residues also indirectly confirmed that PEG-PLGA micelles can promote cellular uptake of the drug. Acute ischemic myocardial model rats were prepared by coronary artery ligation, and then the model rats were randomly divided into six groups: Sham operation group, model group, puerarin(PUE) group, as well as low-, mid-, and high-dose PUE@PEG-PLGA micelles groups. Drugs were given by iv administration 5 min after the ligation. The ST segment changes in the electrocardiogram were monitored; serum creatine kinase(CK), lactate dehydrogenase(LDH), aspartate aminotransferase(AST), and malondialdehyde(MDA) levels were detected and myocardial infarct size was also measured. Both PUE and PUE@PEG-PLGA micelles can reduce the elevated ST segment, reduce serum CK, LDH, AST and MDA levels, and reduce myocardial infarct size. The efficacy of PUE@PEG-PLGA medium and high dose groups was significantly better than that in the PUE group, and the efficacy in PUE@PEG-PLGA low dose group was basically equivalent to that in the PUE group. PUE@PEG-PLGA micelles can greatly improve the cardiomyocytes uptake of PUE, enhance the anti-acute myocardial ischemia effect of drugs, and reduce its dosage.

ABSTRACT

Computed tomography (CT) is considered the most sensitive method for the detection of intraocular foreign bodies (IOFBs).The purpose of this study was to evaluate a new method of 3-dimentional (3D) localization of IOFBs that takes advantage of the anatomical structure of the optic nerve and to assess the clinical outcomes using this new method.Twenty-two trauma patients with IOFBs or suspected IOFBs admitted to our hospital were scanned with multislice CT (MSCT) between July and December 2003.All scanning was performed with a 16-row spiral CT in axial plane using a sequential scanning protocol.During the scanning, the eyeball of the patient was kept stable and was not allowed to rotate internally or externally.Section collimation was set at 16 mm × 0.75 mm.Table feed was 12 mm.Reconstruction index was 0.75 mm.After scanning, the reconstructed images were loaded into a workstation to create the multiplanar reconstruction images with the aid of the 3D software. We compared the localization results with the operative findings.Multiplanar reconstruction images showed IOFBs in all 22 patients.IOFBs occurred in the eyeball of 14 patients,in the wall of the eyeball of 5 patients and in the posterior orbits of 3 patients.Different surgical procedures were designed according to the localization by this new method and all IOFBs were successfully removed. All of these foreign bodies were metallic and the localization of IOFB using MSCT was consistent with that found by operative findings. It was suggested that MSCT is a simple and effective imaging modality for the localization of IOFBs.In our study,we localized the IOFBs more quickly and accurately by taking advantage of the fixed position of the intraocular segment of the optic nerve, and determined the necessary surgical parameters.

ABSTRACT

OBJECTIVETo search for a new method of screening for molecular targets for androgen-dependent prostate cancer.

METHODSWe collected tissue samples and paired serum samples from 3 cases of androgen-dependent prostate cancer (ADPC) treated by surgical resection, and included another 3 samples of benign prostatic hyperplasia (BPH) tissue and normal human serum in the control group. The total proteins extracted were separated and transmembrane by two-dimensional gel electrophoresis, followed by hybridization with the sera of the patients with ADPC and those with hormone-independent prostate cancer (HIPC) as the primary antibodies. The differentially expressed proteins were compared by Western blot, analyzed by MALDI-TOF-MS mass spectrography, and verified by RT-PCR and Western blot following bioinformatic identification.

RESULTSThis modified method exhibited a significantly better effect in displaying differentially expressed proteins, by which 12 differentially expressed protein spots were identified, including Beclin1, glutathione S-transferase P (GSTP1-1), ZBTB7, dihydrodiol dehydrogenase 2 (DDH), enolase (ENO1), glucose-dependent insulin-releasing peptide receptor (GIPR), Mn-superoxide dismutase (MnSOD), phosphoglycerate mutase 1 (PGAM1), amino-peptidyl-prolyl cistrons isomerase (PPIA), and phospholipid-PE-binding protein (PEBP). The mRNA and protein expressions of Beclin1 were significantly down-regulated in androgen-dependent prostate cancer tissues.

CONCLUSIONThis modified serum-guided immunoblotting technique has provided a new method for clarifying the molecular mechanisms of the occurrence and progression of HIPC, in which Beclin1-mediated autophagy may play a key role.

ABSTRACT

Objective To evaluate the potential of a sliding multi-slice (SMS)fast low-angbe shot 2-dimensional (FLASH-2D) sequence for abdominal lesion detection in patients with rectal cancer. Methods Nineteen paired SMS MRI( FLASH-2D sequnce) and MSCT examinations of the whole abdomen and pelvis in 15 patients (four of them were examined twice) with rectal cancer were retrospectively analyzed by two radiologists. While the lesion-based agreement between the two methods and the diagnostic agreement between two observers were tested by means of Kappa statistics, the sensitivities of SMS FLASH-2D and MSCT to detect liver metastases, lymph node metastases and bone metastases were calculated.Standard of reference consisted of a consensus evaluation of SMS,MSCT and all available follow-up examinations after a period of 6 months. Results Using SMS FLASH-2D cumulatively, both observers detected 56 of all 60 lesions respectively resulting in same sensitivities of 93.33% (56/60).Using MSCT,both observers detected 50 lesions, resulting in same sensitivities of 83.33 % (50/60) respectively. The sensitivities of SMS FLASH-2D to detect hepatic metastases were 97.44% (38/39) and 100% (39/39) for both observers respectively, compared to 100 % (39/39)and 100% (39/39)for MSCT.The sensitivities for lymph node metastases were 85.71% (12/14) and 71.43% (10/14) for SMS FLASH-2D compared to 78.57% (11/14) and 71.43% (10/14) for MSCT.The sensitivities for bone metastases were 85.71% (6/7) and 100% (7/7) for SMS FLASH 2D compared to 0(0/7) and 14.29% (1/7) for MSCT.Conclusion SMS FLASH-2D imaging and MSCT exhibit equal ability of detecting lymph node and liver metastases, but SMS FLASH 2D seems better than MSCT in dectecting bone metastases.

ABSTRACT

This study investigated the changes of CD4+CD25+ regulatory T cells (Tregs) in peripheral blood of patients with hepatocellular carcinoma before and after transcatheter arterial chemoembolization (TACE). The proportion of CD4+CD25+ Tregs among CD4+ T lymphocytes in peripheral blood of 33 patients with hepatocellular carcinoma was determined by flow cytometry before, 1 week and 1 month after TACE. And 25 healthy volunteers served as control. One month after TACE, the patients were divided into two groups: 22 in group A, who were in stable condition or getting better;and 10 in group B, who were deteriorating. One patient died and was excluded. The results showed that the percentage of CD4+CD25+ Tregs among CD4+ T lymphocytes did not significantly change in the 33 patients 1 week after TACE as compared with that before TACE, however, the difference was significant (P<0.01) between the patients with hepatocellular carcinoma and the healthy subjects. The percentage of CD4+ CD25+ Tregs among CD4+ T lymphocytes in group A 1 month after TACE was decreased significantly in comparison with that before and I week after TACE (P<0.01), whereas, that in group B was increased significantly 1 month after TACE (P<0.01). It was concluded that patients with hepatocellular carcinoma had a higher proportion of CD4~CD25+ Tregs in peripheral blood. TACE did not significantly affect the level of CD4+ CD25+ Tregs within short time (such as 1 week). The proportion of CD4+CD25+ Tregs in peripheral blood 1 month after TACE was related to the prognosis of hepatocellular carcinoma.
